How to complete the survey
The bi-annual survey has two parts. Part 1, the Survey Form, gathers information such as company history, employee average compensation, revenue, and asks for your feedback on Incubator programs and facilities. Part 2, the Employee Information Worksheet, collects individual employee demographic data,vehicle information, and communication preferences.
To submit Part 2, the Employee Information Worksheet, save the file as "Company Name_January 2017 Employee Worksheet" and send as an email attachment to firstname.lastname@example.org.
Surveys should be complete and submitted no later than Friday, January 20th, 5:00PM, EST.
